Online Contest W/ Entry Fee In Florida Pitfalls

Looking to create an online contest based in Florida. The prize will be a logo/visual design package. Contest duration has not been determined, but likely will run for a month. To enter one must:
1) Fill out and submit our own form
2) Include a brief essay on their organization and why it wants/needs a new logo and visual identity
3) Include an entry fee (the amount has yet to be determined)
From what I can see it is legal to run such a contest in Florida, but looking for two things:
1) Sample terms and conditions for such a contest
2) Any other pitfalls we'd want to beware of.
