Hi. I need help on some survey questions relating on the effect of online and offline shopping towards consumer behaviour. Can you suggest me some?

Perhaps that ought to be consumer behavior as it pertains to the activity of shopping, not the other way around.

Where do people shop?

What categories do they buy in?

Does store layout or webpage design influence buying decisions?

How often for online as opposed to offline and vice versa?

What do they spend?

Do they buy more if offers are presented at point of payment?

Do they collect and redeem coupons or discount codes?

Would they buy more often, at higher price points if they were offered discount coupons and codes?

What is the objective of the research? What are you trying to learn? How will the results be used?

Without knowing the answers to these questions it's impossible to suggest meaningful survey questions. Who is commissioning/sponsoring the research, and what do they hope to learn from it?
